Dose anyone know where I can find information on how to set up a contact us form? A good example is http://www.haircutadvice.com/contact.php

 You can perform a simple search with "contact form" (select php scripts), in hotscripts.com there's a lot of contact form scripts for free and commercial.

 the question is why? Why do you need it for?

you got a service to provide people?

A simple contact form is just a single webpage will your email address.
Do you have the time to entertain spam email?

Unless you are providing a helpdesk support else I dont see any reason you need to waste time on this.

Spend time to get traffic to your site. Grow the list. Opt in list is more important.

Everything you have on your website must have a good reason.
